utilizes cross system collaboration of existing community providers to connect families and caregivers with resources that go hand in hand to support the family’s needs.

HOW: Help Me Thrive Family Navigators support community providers and case workers. Family Navigators are here to brainstorm and collaborate with providers on resources available in the community or for coaching support. Family Navigators are fully trained to locate resource options within and outside the FINDconnect database.

CONNECT: Providers are welcome to schedule time to connect with Help Me Thrive Family Navigators to present their program’s services for youth 6-18 or to receive counsel on any resource challenges they may have.
